Nikamma is a film about a young man named Aditya, who is jobless but possesses the unique ability to have a photographic memory. His older brother Raman adores him and takes care of all his needs until he marries Avni, who becomes the center of his attention. Feeling ignored and resentful, Adi leaves home and lives with his uncle for over a year. Upon returning, he learns that Raman is moving to Bengaluru while Avni is relocating to Dhamli. The film revolves around Adi's complex relationship with Avni, his sister-in-law, and the challenges he faces in reconciling with his brother and finding his place in the family. Raman asks Adi to accompany Avni to Dhamli, but Adi is not very keen on doing so. Avni gets a high-ranking job at the RTO, and she makes Adi do all the household chores which irritate him. One day, Adi gets so frustrated that he decides to run away but then meets Natasha, who proposes to him right away. Adi is surprised but decides to stay back and have an affair with her. As they get to know each other, Adi learns that Natasha is Avni's cousin and had fallen for him when she saw him at Raman and Avni's wedding. Natasha then moves in with Avni so that she can be closer to Adi, who she is smitten with. Avni is unaware that Adi is having an affair with her cousin, Natasha. When she finally discovers their relationship, she sends Natasha away, which makes Adi even more upset. Meanwhile, another person who is angry with Avni is Vikramjit Bisht, who is running an illegal taxi service and has killed 40 people by setting a bus on fire. Avni decides to take action against Vikramjit and seizes his illegal cars. This makes Vikramjit so angry that he goes to RTO to kill her. At the same time, Adi meets his uncle who tells him about the sacrifices that Avni has made for him. What happens next forms the climax of the movie.
